Cowichan Collaborative Health (CCH) is a new muliti-disciplinary health care clinic in central Duncan, BC on Vancouver Island. The service delivery model of this clinic is built on compassionate, longitudinal team-based care.
CCH launched January 2025 with Family Physicians, a Licensed Practical Nurse, a Paediatrician, a core team of office administration, patient partners, a Social Worker and a clinical Pharmacist. We will soon be adding more Family Physicians, a Sleep Medicine Specialist and a Clinical Counsellor to the team. There is a lab and pharmacy on the premises. The clinic culture involves team-based, compassionate, whole person care and is based on a model of compassionate leadership as mentored by Shawn and Heather Atleo of “Team Atleo” where strong relationships between staff and providers are fostered through non-violent communication.
